Fundamentals of Oncoplastic Breast Surgery

This hybrid course is for general and breast surgeons who
have never performed oncoplastic surgery. A review of fundamental elements of
oncoplastic breast surgery will include applied anatomy, imaging, patient
assessment and patient selection. The online didactic lectures will cover basic
and often used oncoplastic techniques to improve the aesthetic outcome of
partial mastectomies; incision placement, parenchymal closure, intramammary
flaps, de-epithelialization, crescent mammaplasty, and donut mammoplasty. Office
management pearls presented include assembling and developing a
multidisciplinary team and building functional referral networks. During
hands-on surgical skills lab, participants will perform skin marking, glandular
displacement techniques for intramammary flaps, aesthetic incision placement,
and periareolar, crescent and donut mammaplasty. Also, participants will learn
simple techniques to improve lumpectomy results and management of excess skin,
"dog-ears", and tissue during simple mastectomy without reconstruction.
Course Objectives
At the conclusion of this course learners will be able
to:
1. Understand and incorporate knowledge of breast
aesthetic assessment, macromastia and ptosis categorization as part of breast
cancer surgical planning. Understand surgical outcomes based on tumor
location.
2. Better understand strategies for aesthetic scar
outcomes, options for incision placement based on cancer presentation and breast
shape, and surgical techniques to preserve breast contour and minimize scaring.
Learn how to improve visualization and exposure of tumors with aesthetic scar
placement.
3. Apply techniques for donut /benelli mammaplasty with
extension to address and access all quadrants of the breast.
4. Understand skin management techniques to produce
aesthetic flat mastectomy, how variations in patient anatomy and body habitus
impact choice of approach, and coding of these approaches.
5. Gain a working knowledge of level I and II oncoplastic
techniques. Understand HPPIA compliant patient photographic image documentation
and storage, and the role of these resources at diagnosis, in surveillance, and
in preoperative counseling.
